WP 7 will coordinate the implementation phase of four pilot sites: (a) City of Rijeka, Croatia; (b) Treviso, Italy; (c) Rotterdam, the Netherlands; and (d) Camborne/Redruth, Cornwall, United Kingdom.
These 4 pilot sites will implement and contribute to the evaluation of the integrated SEFAC model, based on the preparations in WP4, WP5 and WP6. In each pilot site, there is a local pilot site coordinator.
Objectives
- Implement the innovative SEFAC’s template for addressing chronic diseases prevention and intervention based on local social engagement community interventions in 4 European areas (Rijeka; Treviso; Rotterdam; Cornwall).
- Produce the data that allow the evaluation of SEFAC’s impact in terms of effectiveness, and cost-effectiveness, and prepare for its sustainable exploitation.
